Freedom Physiotherapy and Wellness Centre has existing contractual agreements with all the major insurance providers. In order for you to receive treatment with a member of our physiotherapy team we kindly ask you to ensure you have your Provider Membership Number and Individualised Authorisation code when calling the clinic to make your first appointment. Without this information you may be eligible for costs if this information is not obtained prior to your first appointment. In the majority of cases, or unless you advise us not to, we will send your treatment bills directly to your insurance provider. Therefore it is vitally important you are able to provide us with the above information. It is your responsibility to contact your insurance provider to gain authorisation for the treatment you intend to receive. If you are unable to provide us with this information, we will ask you to settle the bill in clinic and we can then provide you with a receipt so you can then claim this back from the Insurance Provider yourselves. In the event your private medical insurance prevents you from having your treatment covered, the self funding rate will then be applied. Any shortfalls or excesses in your policy are your responsibility and Freedom Physiotherapy and Wellness Centre will not be responsible for any charges imposed on you by your insurance company for failure to attend or missed appointments.
